Re:Ms Sheila Cartwright, aged 69 years
I am writing to refer Ms.Sheila Cartwright who requires follow up care and support . She has been diagnosed with unstable diabetes mellitus and a small left foot ulcer. She is being discharged today.
Ms Cartwright was admitted to hospital on 14 April for the stabilization of non insulin dependant diabetes mellitus and management of foot ulcer. Daily saline wound dressing was provided for her and commenced on antibiotic therapy(augmentin duo forte, 125mg, two times in a day. She is on glibenclamide, 5mg, daily, which needs to be continued and metformin , 800 mg, three times daily. In addition to that, she experiences periodic difficulties while taking her oral hypoglycemic agents.
Socially, Ms Cartwright is single and lives alone in her own house. She has no close relatives and friends. Since retirement she consumes more alcohol and less diet . She has had diabetes for 9 years.
It would be appreciated if you could visit Ms Cartwright and do daily saline dressing and observe the wound. Kindly monitor her diet intake , blood sugar level , alcohol intake , medication compliance and life style modifications. Please note that she needs to introduce an income producing and social activities . It is beneficial to begin an exercise programme.
