Re: Miss Cassandra Kinser, aged 32
I am referring Miss Kinser, who is diagnosed with lip laceration and urinary tract infection. She requires further medical management and treatement from your facility and following discharge today.
On 17 January Miss Kinser is admitted to the hospital with complaints of alleged assault by boy friend, abdominal pain, burning while urination for the last many days vomiting and diarrhoea as reported. The lip
laceration condition as treated with cipro 500 mg orally twice daily continued for 6 days, clindamycin 300 mg orally daily for 10 days, oxycodone one orally very 4 hours and peridex mouthwash 10 ml swish and spit after
meals and before bed was commenced.
Miss Kiner is unmarried and lives with his boyfriend. She is sexually active and not followed protective devices. Socially he has a habit of smoking, alcoholism as well as denies illicit drugs.
The medical history revealed had a schizophrenia was reported.
Presently, prognosis was observed in constiutional, cardio vascular system as well as respiratory system and home condition was good.
Appointment is sheduled at sexual health clinic. Based the aforementioned circumstances, monitor antibiotics and provide appropriate treatement for urinary tract infection it would be
profoundly beneficial.
