Re: Mr John Alex Fredrick, DOB: 18 June 1987

Dear Ms Forest,

I am referring Mr Fredrick, who has an impaired gait due to stroke, requires home visits and appropriate physical therapy for his speedy recovery.

On 12th July 2017, he presented with the complaint of serious injury in the left knee following extreme headache while descending the stairs. Therefore, daily dressings and personal hygiene has been initiated by Raymound Health Care Agency. On subsequently visits, the wound healed was excellent without infections.

During the follow-ups, it was reported that he was able to ambulate a short distance with support of his wife and progressing well. Unfortunately, he was complaining usual pain. On 19th July 2019, a walking aid has been recommended. In addition, his wife has requested for physiotherapy care to improve the mobility.

Based on the aforementioned circumstances, regular home visits and assist him to perform appropriate exercises for retaining his normal routine activities.
