Re: Mr Charles Gardiner, aged 63
I am writing to upadte you the current health status of Mr charles who is requires ongoing care recuperating from angina.
On 12 November Mr Gardiner admitted to our hospital with complaints of pain, discomfort and tightness acros the front of the chest. On examination blood pressure was elevated that is 170/11 mmof hg was noticed. In
addition the myocardial perfusion scintigraph revealed that angina was confirmed. Therefore he underwent a surgery on 25 October 2011.
Mr Gardiner is lives with his son. He is suffering from sensory vision within normal limit glasses and heard of hearing as reported. However therewill be difficulty in breathing continent of bowel and incontimento bladder were observed.
Therefore for supervision assist in daily living activity so son was provided primary care giver. He had a habit of smokes and also were disposable undergarments.
Based on aformention circmstances, provide proper ongoing care it would be profoundly beneficial.
