Dear Ortner,

I am referring Mr Nick Daniel, who is recuperating after a hip replacement surgery, requires physical therapy and appropriate education regarding proper use of a walker following the discharge today.

On 13th May 2019, he presented with of severe pain in the right hip and knee and decreased internal rotation and flexion in the right hip. Therefore, osteoarthritis in the right hip was diagnosed. In addition, a x- ray was performed which revealed degenerative changed consistent with the osteoarthritis. Consequently, indomethacin 50mg has been commenced.

Subsequently, it was reported, he was non- compliance with medication; therefore, increased pain, decreased range of movement, unable to bear weight and exacerbation in the right hip were observed. As a result, indomethacin 50mg was recommenced. However, similar complaints were reported again, was admitted to the hospital and underwent surgery on 19th July 2019.

Postoperatively, physiotherapy has been initiated to improve the mobility and suggested that risk of dislocation and discomfort was observed.

Based on the aforementioned circumstances, education regarding weight bearing exercises and to be caution while using stairs folloeing the complete recovery process. Demonstrate about how to use a walker or crutches is appreciated.
