Re: Mr Walter Pitman, aged 69
Dear Dr Laricrell,
Mr Walter Pitman, who has been persistent an elevation of blood pressure, is being referred for assessment and appropriate management from your service.
On 1st July 2018, he was admitted due to the deep lacerations in the lower left arm following a cut injury. Therefore, the wound was cleaned and sutured. In addition, prophylactic IV antibiotics course completed followed by oral antibiotics has been commenced. Subsequently, advised to visit The Nurse-Led Outpatient Clinic for further wound care following the discharge.
Initial visit, severe pain with mild hematoma, and pain score 8/10 were reported. Additionally, a significant raise of blood pressure was observed. On 11 July 2018, the blood pressure was 182/ 80 and 194/ 86 mm of Hg in the sitting and supine respectively were noticed. However, the wound healing process was excellent without any signs of infection. On todays’ follow-up, the blood pressure has been remained elevated in the sitting and supine positions 190/84 mm of hg and 196/86 mm of hg respectively. Therefore, orthostatic hypertension has been suspected.
Based on the aforementioned circumstances, a cardiac assessment for hypertension would be extremely beneficial for managing his blood pressure.
