Dear Mrs Penny,
Re: Ms Sheila Cartwright, aged 69
Ms Sheila Cartwright, who has been diagnosed with uncontrolled diabetes mellitus and infected foot ulcer requires continuous monitoring and care followed by her discharge today.
On 14 April 2017, Mr Cartwright was admitted to hospital with the above mentioned complaints for which, she has been taking Tab. Metformin 800mg three times daily for the past 9 years. Besides, diabetic foot ulcer on her left foot treated with Aigmentin Duo Forte 125mg twice daily.
During hospitalization, Tan. Glibenclamide 5mg daily has been Commenced along with other tablets which Saline dressing and wound assessment done on daily basis and she is recuperating from her condition.
She is single, retired administrative officer lives with her small pension. No close relatives on friends to look after her. She has habit of taking alcohol which is increased after retirement observed non complaince with medication.
As per aforemetioned circumstances, monitoring of adherances medication and checking of blood sugar level is profoundly beneficial. Recommended for daily exercise and social activities along with healthy diet and reduction of alcohol is essential. Co-operation with community worker and social worker has been suggested for her future life.
