Dear Ms Gold,
Re: Mr George Gale, DOB: 24 April 1936
Mr George Gale,who has been diagnosed with urinary tract infection is being referred to your facility for provisional care and definitive management following his discharge today.
On 10 May 2021,Mr Gale was admitted to Old town Hospital with complainants of fever and disorientation along with an episode of fall at home. On examination, he was confused and vital signs were elevated such as temperature 38.1 C, blood pressure 150/80 and respiration rate 26.Urinalysis report confirmed the diagnosis. During hospitalization, he was treated with intravenous amoxicillin 750mg three times daily.
Mr Gale, a retired retail manager is a widower and lives alone in a level 2 flat where no lift facility. He has a son who works abroad and has no family nearby. He interacts with neighbours and drinks socially. He smokes 10 cigarettes daily. He has had osteoarthritis and hypertension since 2003 and 2009 respectively for which he is on paracetamol 500 mg two tablets four times and felodipine 5 mg daily.
Pertaining to the above, it would be appreciated if you could take over the care of Mr Gale and ensure adequate physical activity. It is profoundly beneficial to perform exercises thirty minutes daily in order to facilitate his markedly diminished mobility. A detailed evaluation regarding his independent living circumstance is immensely significant as he still experiences episodes of confusion as well as lives alone without any family support.
