Re: Ms Iris lui, 94 years
Ms Iris lui, who has been diagnosed with urinary tract infection, renal insufficiency and dognerative joint disease is being transferred back to your facility for coninuing care and appropriate management following her discharge today.
Ms Lui was admitted on 15 February 2011, with complaints of chronic anemia, hypertension and volume depletion. On examination, she appeared lethargic. During hospitalisation, she was treated with intravenous fluids and subsequently, renal function has improved. Blood culture report was found negative. Currently, she has made a good prognosis; however, assistance is required in showering, dressing as well as ambulating. She walks with the support of frame and stick .
Ms Lui's daughter visits her regularly and supports her in daily living activities and provide her food. She is reserved; therefore routine visits of her daughter is immensely significant.
Pertaining to the above details, it would be appreciated if you could take over the care of Ms Lui and monitor her medication regimen. Her current medications are Nerium 40 mg, potassium 10 mEq and Diclofenac twice daily. Lat 40 mg tablets and Augmentin 875 mg twice daily need to be continued for one more week. Providing assistance in showering, dressing along with mobilisation is profoundly beneficial.
