Re: Ms Sheila Cartwright, 69 years


Ms Sheila Cartwright, who has been diagnosed with unstable diabetes mellitus requires care and management for healthy lifestyle after her discharge today.


Ms Cartwright was admitted to the Freemont Community Hospital on 14 April 2017 following uncontrolled diabetes with small foot ulcer. Her diabetes has been treated with oral anti- hyperglycemic agents (Glidenclamide 5mg daily, metformin 800mg tds ).Additionally, her foot ulcer has responded to antibiotic therapy (Augmentin Duo Forte 125mg orally twice a day)and daily saline dressing. Furthermore, Ms Cartwright will require ongoing management and monitoring of the wound for complications and medication requime.


Ms Cartwright is retired. She drinks alcohol in excess with low dietary habits following her retirement.


Based on the aforementioned circumstances, it would be beneficial if you could check her blood levels, alcohol intake and diet pattern. It will also be helpful to introduce her to some ways of expanding her social activities and engaging in moderate exercises. A referral to community social worker also arranged for Ms Cartwright healthy lifestyle.
